FAQ
What is Emera™, and how is it different from a survey or callback program?
Emera is a co-designed, empathy-led post-discharge solution built to identify emerging risk after a patient goes home, not to measure satisfaction after the fact. A survey asks how care went once the window for fixing anything has closed. A simple callback program checks a box. Emera is different on both counts: every call is made by a live human caller, not an IVR system, and we personally call 100% of discharged patients within 24-48 hours, listening for what’s actually going on and moving any concern through a clear path (Emerge, Mobilize, Escalate, Resolve, Advance) until it’s resolved.
How does Emera integrate with our existing discharge workflow?
Emera is designed to work alongside your existing readmissions and patient experience initiatives rather than replace or restructure them. It doesn’t require new workflows, new documentation steps, or additional tasks for discharge planning or nursing staff. Outreach happens through Emera’s own process, and only issues that need clinical or operational attention get routed back to your team, already identified and prioritized, instead of surfacing as a stack of undifferentiated calls.
Who owns follow-up when a concern escalates?
Emera is a co-designed, empathy-led post-discharge solution built to identify emerging risk after a patient goes home, not to measure satisfaction after the fact. A survey asks how care went once the window for fixing anything has closed. A simple callback program checks a box. Emera is different on both counts: every call is made by a live human caller, not an IVR system, and we personally call 100% of discharged patients within 24-48 hours, listening for what’s actually going on and moving any concern through a clear path (Emerge, Mobilize, Escalate, Resolve, Advance) until it’s resolved.
We use another vendor for patient experience, can we still implement Emera?
Yes. Emera operates independently of whatever patient experience or HCAHPS vendor you already use. You don’t need to change vendors, integrate systems, or coordinate between the two to implement Emera. It’s designed to work alongside your existing patient experience program, not replace it or require a switch.
How is outreach delivered across languages and cultural contexts?
Outreach is delivered in multiple languages and is culturally responsive, so patients are met in a way that feels understood rather than processed. PRC’s call center team is multi-lingual.
